Job Description/Responsibilities:
Assist our physical and occupational therapist with providing physical and occupational treatment including exercises and activities with individuals.
Performs a variety of individual care duties including personal care and provides training to the individuals in a variety of areas including physical and occupational therapy.
Follows verbal and written direction in constructing, repairing, or modifying adaptive and/or functional equipment, uses appropriate tools for the assigned task.
Notifies appropriate individual of any problems regarding adaptive and/or functional equipment
Observe and maintain awareness of the individuals behavior and physical whereabouts to ensure their security and safety.
Monitors individuals for any behavior which could be dangerous to self or others.

*Additional Information - Selected applicant must meet the following standards:

1. Must pass a pre-employment background check (which includes criminal records check, driver's history, and verification of work history) and a post-hire federal background investigation.

**High School and College Degrees Must Be From an Accredited Institution**

High school diploma or GED AND One year of experience in a related field OR One year of experience at the lower level Health Aide 2 (HCS021) or position equivalent.
