Paygrade/Salary: G-$37,822.05, H-$41,604.26 or I-$45,764.68

General Nature of Work
· Participates in the home visiting and remote services to eligible pregnant and postpartum women in Dougherty, Terrell, Worth, Mitchell, Colquitt Thomas, and Grady.
· Under supervision, makes assessments and provides psychosocial support and education to women and families. Makes resource referrals as necessary.
· Documents in medical record/electronic medical record
· Provides case management-type services, coordinates with providers of services, and works collaboratively with community agencies and providers
· May participate in health fairs, serve on committees, make presentations, and respond to Public Health Emergencies
· Provides outstanding customer service and maintains HIPAA standards related to Privacy and Security
· Responds to Public Health Emergencies
Minimum Qualifications
Master's degree in a related field from an accredited college or university AND eligibility to be licensed OR Bachelor's degree in a related field from an accredited college or university AND Two years of experience in social service delivery. (Please mail or bring a copy of College Transcript or Diploma to Interview—must have official transcript prior to start date). This position requires the ability to lift up to 45 lbs. and the ability to bend and reach up.
Preferred Qualifications
· Previous employment with any state’s Department of Public Health or a County Board of Health
· Experience conducting home visits and patient home assessments
· Experience with Case Management and Community Outreach
· Bilingual (Spanish and English).
A State Merit Application for Employment must be received by this office no later than the deadline. Highlight any experience related to the Preferred Qualifications. This position may require driving company vehicles (Valid driver’s license required) and working some evenings and weekends during special events such as health fairs and responding to disasters. Selected applicant must consent to a motor vehicle record assessment. Points should be less than 10 and/or not negatively impact insurance rates for the agency. Drug screen, criminal background check and immunization records may be required. If selected, male applicants between 18 and 26 years of age must present proof of Selective Service Registration.
