Description:

JOB PURPOSE:
Registration and completion of fetal birth and fetal death certificates in accordance with regulatory standards. Maintains and monitors production performed by the transcription service. Performs chart maintenance activities, including chart analysis based on medical staff bylaws, DNV, TJC, or other regulatory agency documentation requirements and assigns deficiencies to clinical providers for resolution. Investigates accounts missing documentation needed to complete coding and billing.

KEY RESPONSIBILITIES:
1. Register births within 5 days of delivery.
2. Register fetal deaths within 72 hours of delivery.
3. Interview patients and communicate with State officials and others in preparation and completion of fetal birth and fetal death certificates.
4. Act as a liaison between hospital and outside transcription services by monitoring work and expected turnaround times and provide feedback as needed.
5. Monitor pended reports and ensure they contain the proper patient, account and document identifiers.
6. Monitor work queues for faxes and copy requests and distribute documents as appropriate.
7. Ensure questionable or problem records/reports are investigated within 24 hours.
8. Conduct concurrent and post-service reviews of medical record documentation and perform quantitative and qualitative analysis to identify potential chart deficiencies (e.g. missing history and physicals, physician signatures, etc.).
9. Assign deficiencies to appropriate clinical or administrative resource; track progress towards resolution; reassign deficiencies as necessary.
10. Conduct weekly audits of incomplete records for providers pending suspension.
11. Provide health information in accordance with hospital and departmental policies.
12. Communicate professionally with others.
13. Daily focus on attaining productivity standards, recommending new approaches for enhancing performance and productivity when appropriate; report productivity weekly to immediate supervisor.1

15. Adhere to Piedmont organizational policies and procedures for relevant location and job scope.
16. Complete and/or attend mandatory training and education sessions within approved organizational guidelines and timeframes.
17. Other miscellaneous duties as assigned.

Qualifications:

MINIMUM EDUCATION REQUIRED:
High School graduate or GED.

MINIMUM EXPERIENCE REQUIRED:
Two (2) years of experience in health information management in an acute care setting or graduate of an HIM program.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
None.

ADDITIONAL QUALIFICATIONS:
Medical transcription experience preferred.
Prior experience using Epic is desired.
Knowledge of medical terminology.
