Heart Transplant Coordinator

Description:

RESPONSIBLE FOR:

The Heart Transplant Coordinator is responsible for managing the care of heart transplant recipients throughout the post-surgical phase. This role involves patient education, coordination of care with a multidisciplinary transplant team and ensuring compliance with regulatory and transplant program guidelines

Key Responsibilities

Provide ongoing-education to heart transplant recipients and their and caregivers regarding transplant protocols and best practices.

Conduct educational sessions for healthcare staff on transplant-related topics.

Monitoring patients through a surveillance protocol for organ-rejection

Coordinate communication between cardiologists, surgeons, infectious disease specialist, pharmacists, social workers, dietitian and other members of the multidisciplinary transplant team.

Ensure continuity of care by facilitating smooth transitions between inpatient and outpatient settings.

Maintain accurate and up-to-date medical records in accordance with hospital policies and transplant program regulations.

Ensure compliance with UNOS, CMS and other regulatory bodies.

Participate in Quality Improvement initiatives

Work Environment:

Primarily outpatient. Coordinators will interact with post-surgical inpatients for education purposes prior to discharge.

Fast paced, requiring critical thinking and adaptability.

Qualifications:

MINIMUM EDUCATION REQUIRED:
Graduate from a Registered Nurse Program.

MINIMUM EXPERIENCE REQUIRED:
Two (2) years of professional clinical experience required.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current License in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License.
BLS and ACLS certification required.

ADDITIONAL QUALIFICATIONS:
Cardiovascular focus experience preferred.
One (1) year experience caring for LVAD patients and heart transplant patients preferred.
Leadership qualities including time management, verbal and good written communication skills, listening skills, decision making, priority setting and work organization skills. Must be self-directed and use independent judgment. Effective communication skills and team building skills required. Demonstrate compassionate patient communication.
