The PEM Care Management department is made up of a diverse group of individuals. Each come with their own unique backgrounds, skills, and experiences. The department is comprised of Social workers, RNs, and CMAs. Together we have built a team that can draw on each others strengths and supplement each others weaknesses. We expand each others knowledge and skills through collaboration and coaching, and provide opportunities for growth within the department and organization. We work together as a team every day to expedite the most appropriate and timely discharges for our patients. We work collaboratively with other members of the interdisciplinary team, as well as post-acute providers to ensure continuity of care. We do this and so much more everyday as a team!

RESPONSIBLE FOR:
The Manager of Care Management, under the guidance of the Director of Care Management, provides leadership, coaching and mentoring of the Care Management department professional and administrative staff in order to support and ensure standards of practice for service delivery and organizational performance strategies are consistent with PHCs Mission, Vision and Values.
The manager will also provide Care Management services for hospital patients as appropriate to guide care facilitation, coordination, and transitions of care.

Qualifications:

MINIMUM EDUCATION REQUIRED:
Bachelor's degree from an accredited school in nursing, social work, healthcare administration or other related field required. If candidate is a social worker, Master's degree from an accredited social work program required.

MINIMUM EXPERIENCE REQUIRED:
If RN: Need combination of 5 yrs of Nursing and Care Management exp in a hospital setting. Experience must include 2 yrs of leadership/Management exp in acute care setting. If Social Worker: 5 yrs clinical Social Worker experience, to include 2 yrs of leadership/management exp in acute care setting. Comprehensive knowledge of the health care industry specifically regulatory standards and national practice standards related to discharge planning and transitions of care.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
If candidate is a nurse, then a current registered nurse (RN) license in the state of Georgia (or compact state) is required.

ADDITIONAL QUALIFICATIONS:
If candidate is a social worker, then current unrestricted licensed master social worker (LMSW) licensure in the state of Georgia, preferred.
Certification in Case Management or Social Work, preferred.
