Piedmont Rockdale is a 161-bed, acute care, not-for-profit community hospital in Conyers that has provided compassionate, patient-centered care to Rockdale County and surrounding communities since 1954. We offer 24-hour emergency care, plus most major medical, surgical and diagnostic services.

Responsibilities: The Social Worker strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. Qualifications: Education

• Masters degree from an accredited social work program Required

Work Experience

• 2 years of experience in an acute or post-acute setting Required

Licenses and Certifications

• LMSW - Licensed Medical Social Worker - State Licensure in the state of Georgia Preferred
